Your requisition form will be emailed to you within 48-72 hours of purchase. When you receive this requisition form, print it out and take it to any LabCorp in the area. You can find your nearest LabCorp on their website. Appointments are not required. You will have your blood drawn and a urinalysis done there. Please note: you need to be fasting at least 8 hours before, so it's best to go before you've eaten breakfast in the morning. Your results will be sent to me about 24 to 48 hours after you've had your blood drawn and I will email them to you.
